You can apply by calling 1-866-454-8387 and request an application:

• Provide your name, telephone number, address and an application will be sent to you.
• Applications are mailed on Friday
• If you request an application on a Friday, your application will be mailed the following Friday
• The application must be signed and returned accompanied by a copy of the:
Telephone bill
Documentation showing your eligibility in a qualified State Programs (if applicable)
3 months proof of income (if not eligible from a State Program)
